Hello everyone. My name is Tara. I am the "sister" to Max, a 1 1/2 year old Maso mastiff. I stumbled across this site after searching for info on chylothorax, which my sweet boy was just diagnosed with. The Doctor said he's never seen it in a dog Max's age. After draining 2.5 L of fluid last week and multiple 20cc syringes later, the chest tube finally came out today. Max will have another xray in 5 days. If there is no additional fluid accumulation then his exploratory surgery will be postponed. He's home resting and enjoying time with his doggie sister Maggie (a spoiled rotten pug).

Before Max, my parents and I were proud owners of an Old English mastiff, McBeal, aka McBeef, McBealy, Big Boy. We lost him almost 4 years ago after a long hard fight with bone cancer. Max came into our lives like a ray of sunshine and I'm NOT ready to loose him. Hes a great defensive soccer player and we have many more games to play. Please keep him in your prayers. The next 5 days will be crucial and one heck of a waiting game for a very impatient "sister" .

It sounds like your vet is knowledgable and vigilant. Glad your boy has you to play soccer with!

Hi Tara...chylothorax is a tough diagnosis...I hope he continues to do well and that you have eachother for a long time still!!  Good luck and please keep us posted...

Chylothorax

A chylothorax (or chyle leak) is a type of pleural effusion. It results from lymphatic fluid (chyle) accumulating in the pleural cavity.

Causes

Its cause is usually leakage from the thoracic duct or one of the main lymphatic vessels that drain to it. The most common causes are lymphoma and trauma caused by thoracic surgery. If the patient is on a normal diet, the effusion can be identified by its white and milky appearance as it contains high levels of triglycerides.

The condition is rare but serious and appears in all mammals. In animals, chylothorax usually results from diseases that cause obstruction to the thoracic duct preventing lymph from draining normally into the venous system. Examples include tumors, heartworm disease, right sided cardiac failure, or idiopathic lymphangiectasia.

 Treatment

Since the mechanism behind chylothorax is not well understood, treatment options are limited. Drainage of the fluid out of the pleural space is essential to obviate damage to organs, esp. the inhibition of lung function by the counter pressure of the chyle. Another treatment option is pneumoperitoneal shunting (creating a communication channel between pleural space and peritoneal cavity). By this surgical technique loss of essential triglycerides that escape the thoracic duct can be prevented. Omitting fat (in particular FFA) from the diet is essential. Either surgical or chemical pleurodesis are options: the leaking of lymphatic fluids is stopped by irritating the lungs and chest wall, resulting in swelling and closure of the pleural space. The medication octreotide has been shown to be beneficial and in some cases will stop the chylothorax after a few weeks.[1][2]

In animals, the most effective form of treatment until recently has been surgical ligation of the thoracic duct combined with partial pericardectomy.[3]

Max just had his 5 day follow up Xray post chest tube and I am VERY HAPPY to announce that there is only a small amount of fluid present!! NO SURGERY NEEDED!!! YIPEEE! We are now trying to find Ruitin, which according to our vet isnt the easiest to find. He will need to take 3000mg three times a day. In addition he will be on a stict low fat diet which I am now researching options for. Any suggestions for either would be greatly appreciated. Many many thanks for all the thoughts and prayers.
